If you need special consideration when taking the exam

If you would like to request special consideration when taking an entrance examination due to illness, disability, etc., please be sure to notify us at least one month before the start of the application period for the entrance examination you plan to take.
Please apply to our university Admissions Center.
We will take into consideration the circumstances based on the content of your application, but please note that we may not be able to meet all your requests.
Please note that your application will not affect your results in the entrance exam.
 
 
1. Process from application for exam accommodations to decision on accommodation details
(1) Please prepare all application documents and submit them to our university Admissions Center at least one month before the start of the application period for the entrance examination you plan to take.
(2) After the university accepts your application documents, it will review them based on the contents and documents of your application and determine the details of the accommodations to be made.
(3) Once the details of the accommodations have been decided, we will contact you at the contact information you provided on the “Entrance Examination Consultation Application Form.”
 
2. How to apply
・Send application documents by mail
Prepare the documents specified in "3. Application Documents" and submit them one month before the start of the application period for the entrance examination you plan to take.
Please send it by mail (registered mail or letter pack) to our university Admissions Center.
 
3. Application documents
(1) Application for exam consultation
*Please download the application form below, print it out on one side of A4 paper, and fill in the required information.

(2) Medical certificate or disability certificate (copy acceptable)
*Please submit something that clearly explains the necessity of the desired consideration.
(3) "Notice of Examination Considerations Review Result" (copy) from the National Center for University Entrance Examinations
*Please submit this if you have applied for special consideration when taking the National Center Test for University Admissions.
Please note that we may not be able to accommodate your request in the same way as stated in the "Notification of Examination Considerations Review Results."
 
4. Application Deadline and Shipping Address
(1) Application Deadline
・Please submit your application at least one month before the start of the application period for the entrance examination you plan to take.
*Please be sure to check the "Entrance Examination Guidelines" for the examination dates and application periods for each entrance examination.

5. Important points to note regarding application
(1) If you do not apply by the application deadline (one month before the start of the application period), we may not be able to provide special consideration for your entrance exam.
However, if you are unable to apply by the above deadline due to an unforeseen accident, sudden illness, etc., please contact Admissions Center as soon as possible.
(2) The purpose of the application documents is to confirm the accommodations you would like to request and they have nothing to do with whether or not you are accepted.
(3) Based on the contents of the application, consideration will be given to the type and degree of disability.
However, please note that we cannot guarantee that we will be able to accommodate all requests.
(4) The University Admissions Center may contact you to confirm the details of any requests for examination considerations stated in your application documents.
(5) Regarding considerations regarding the examination, in principle, even in the case of entrance examinations that are held at off-campus locations, the examination will be taken at our university's venue (Musashino Campus).
(6) The information you have submitted in your application documents will be treated with great care as important personal information and will be shared with the entrance examination department and student support department for the purpose of adjusting any considerations for your entrance examination and providing support for your studies after enrollment. However, it will not be used for any other purposes.
Please note that we may contact the contact details you provided in your application documents and the issuers of certificates, etc. to confirm the contents of your application.
