About application and receipt
- We will ask you the following necessary information, so please let us know the information you understand (* is required).

·Student number
・Date of admission ・Date of graduation (completion) or withdrawal (leaving the register) (if the date is unknown, only the year is acceptable)
If you are currently enrolled, please let us know your academic year.
·family name *
For Japanese certificates, please let us know the pronunciation, and for English certificates, please let us know the romaji notation.
It will be issued in the name you used when you were in school. It cannot be issued with a changed surname.
·date of birth *
・Type of certificate to be applied for and number of copies issued *
Graduation certificates written in Japanese usually do not include the date of admission. If you need it for pension procedures, etc., please let us know.
・Presence or absence of strict seal
Certificates written in Japanese will not be sealed in principle. Please let us know if you need it. In principle, all English certificates must be sealed, but if you do not need them, please let us know.
·telephone number * - Please bring your driver's license or other identification card to the Academic Affairs Office Affairs Center on the 1st floor of Building No. 1 on or after the administrative day two days after the date of application. A personal seal is not required, but please let us know that you have already applied by phone.
Please note that it is not possible to change to receive by mail. If you would like to receive it by mail, please use <application at the counter> or <application by mail>. - Please fill in the necessary information on the form "Certificate Issuance Request" provided at the counter and submit it to the counter together with the fee.
